To Register with the Practice
Please pick up a registration pack from reception.
When returning the completed forms you will need to bring two forms of identification which clearly shows your current address, such as a bank statement or utility bill.
When you register it will take a few weeks before your medical information can be forwarded to us. In the interest of accuracy and safety we ask that you give us as much information as you can on the form provided by the surgery and that you make an appointment with the health care support worker for a new patient check, all adult patients are required to have this health check. You will not be registered until you have attended this appointment.
If you would like letters or information/future communications in an alternative format, eg large print or easy read, or if you need help with communicating with us, eg because you use British Sign Language or need an interpreter, please let us know. You will be registered as a patient of the Practice. You may see any Doctor within the Practice for any of your appointments but if you are receiving ongoing treatment for specific problems we suggest you try to see the same Doctor each time.
